Overview
CareBridge is a concept remote patient monitoring (RPM) and telehealth platform for US cardiology, endocrinology and primary-care practices. Connected blood pressure cuffs, glucometers and scales stream readings into a clinician portal that triages alerts, tracks care plans and documents the time needed for RPM billing.
The challenge
RPM programs help practices manage chronic conditions between visits, but they create a flood of device data, manual time tracking for billing and a real risk of missing urgent readings. Practices also need any vendor to handle protected health information (PHI) in line with HIPAA and to sign a Business Associate Agreement.
Our solution
We designed a portal that ingests device readings automatically, applies per-patient thresholds and routes only out-of-range or missed readings to nurses as prioritized alerts. Every review, message and call is time-stamped against the patient, producing the documentation practices need to support RPM billing codes. Patients use a simple mobile app to see readings, message their care team and join video visits.

Built for US compliance
The architecture follows HIPAA Security Rule safeguards: encryption in transit and at rest, role-based access, unique user IDs, automatic logoff, detailed audit logs and hosting on HIPAA-eligible AWS services under a BAA. Softsols signs a BAA with practices for production work.
Technology stack
React web portal, Flutter patient app, Python (FastAPI) services, PostgreSQL, HL7 FHIR integration layer, AWS (HIPAA-eligible services) with infrastructure as code.
